The Patient Safety Culture Survey graphically shows attitudes towards patient safety. 

The shared norms, beliefs, attitudes and assumptions - or the way we do things around here - significantly impacts on patient outcomes. It is people who make the difference not only to the patient experience but also to the entire patient journey.


Key Thinking

Much of the work that has been done around the culture of patient safety has originated in the nuclear and aviation industries. NASA research found that over 70% of the primary cause of accidents was attributable to human error.

Human performance does not exist in a vacuum, and there is growing consensus that the quality of care must be investigated within the context of teams and their work environments, leadership skills, communication and decision making. Improved patient safety is high on the international agenda and managing organisational culture is a lever for healthcare improvement.
